There are several mistakes that are made on Hollywood sets in almost every blockbuster film. Most of these mistakes are minor but somehow still end up on screen and gun owners get to facepalm as a result. 

Few of these mistakes are fatal and tragedies like the one on the set of Rust, sadly though, they do happen.
